People with chronic conditions and complex care needs may be referred by their GP to our centre under an Enhanced Primary Care (EPC) program. Whilst a gap fee is payable to the clinic we are able to bulk-bill a significant proportion of your consultation fees directly to Medicare.
Eligibility criteria include:
• a chronic medical condition that has been (or is likely to be) present for six months or longer, inclusive of (but not limited to);
– asthma
– cancer
– cardiovascular disease
– diabetes
– musculoskeletal conditions
– stroke
• patients who have complex care needs who require ongoing care from a multidisciplinary team consisting of their GP and at least two other health or care providers.
